1)  My loved one doesn't need immediate attention, but her health is declining and she probably will in the future. Being that I live in another state, I can't be as available if there is an emergency. How can ElderCare help me?

ElderCare Services can provide a geriatric care manager to meet your loved one and prepare a preliminary evaluation. This will be a face to face contact noting emotional, cognitive, and physical arenas, as well as medical and social supports.  Then, if a pre-crisis or crisis occurs, the geriataric care manager can be prepared to spring into action upon short notice.

2)  I live near my loved one but I don't know what to do about his increasing memory decline, especially when it comes to eating and taking medication. Due to my hectic schedule, I can't visit him as much as I'd like to. Can a geriatric care manager help me figure out how to keep him safe, while keeping me involved as well?

ElderCare Services provides geriatric care managers that work with you as a team — we "share the care."  The geriatric care manager can be an active participant, or act as a guide to assist you as problems arise, or before they get worse.

3)  How long does it take to put a care plan together?

After a proper home assessment/evaluation, a geriatric care manager can usually formulate an individualized care plan that very same day. If there is an ongoing case, the geriatric care manager can continually re-evaluate the plan as needed.

4)  Does Medicare pay for geriatric care management services?

Unfortunately, Medicare does not pay for geriatric care management. The only insurance available to cover geriatric care management involves particular long-term care policies. More and more long-term care companies are reimbursing geriatric care management, as it is an intervention that keeps emergency room visits, hospitalizations, and nursing home placements down to a minimum. Check with your individual carrier to find out which options are available to you.  

5)  I have concerns about hiring certain caregivers. How can a care manager help me with this?
